As the final step, we will send an email with instructions detailing how you can provide proof of written permission signed by the user demonstrating you have been authorized to make this request. Written permission must include the user’s first name, last name, email address, and authorization for you to act on the user’s behalf in making a request. If the user has provided you with power of attorney pursuant to California Probate Code sections 4121 to 4131, you may instead provide a copy of an executed valid power of attorney naming you as the user’s authorized agent.
